Help paying for a walk-in tub

Medicare, Medi-Cal, VA grants, taxes and local programs in Sacramento County, and what each one really covers.

Does Medicare pay for a walk-in tub?

No. Medicare treats a walk-in tub as a home improvement, not medical equipment, so Original Medicare won't pay for it even with a doctor's note. Some Medicare Advantage plans offer small home-safety benefits (grab bars, bath seats), but we haven't seen one cover a full tub. It's worth calling your plan to ask anyway.

Medi-Cal

Through CalAIM, some Medi-Cal managed care plans offer home modifications (called "environmental accessibility adaptations") for members who qualify. Not every plan offers it, and approval is case by case. If you or your parent is on Medi-Cal, ask the plan or a case manager whether bathroom modifications are covered.

VA benefits for veterans

  • HISA grant (Home Improvements and Structural Alterations): up to $6,800 over your lifetime for a service-connected condition, or up to $2,000 for a non-service-connected condition. Apply through your VA health care team, and get approval before any work starts. The VA won't reimburse work that's already done.
  • SAH and SHA grants are larger grants for veterans with certain severe service-connected disabilities. For fiscal year 2026, the limits are about $126,526 (SAH) and $25,350 (SHA).

If you're applying for HISA, tell us at the estimate. We'll write the quote the way the VA needs it.

Taxes: counting it as a medical expense

IRS Publication 502 allows some home changes made for medical reasons to count as medical expenses, including bathroom modifications for someone with a disability. The amount you can count is usually the cost minus any increase in your home's value, and accessibility changes often add little or no value. You have to itemize, and only medical expenses above 7.5% of your income count. Talk to your tax preparer before you rely on this, and keep the invoice and any doctor's recommendation.

Local help in Sacramento County

  • Rebuilding Together Sacramento, (916) 455-1880, does free safety work for qualifying low-income seniors and people with disabilities, mostly grab bars, ramps and repairs. A full tub replacement is usually beyond what they do, but they're worth calling if cost is the main barrier.
  • Agency on Aging Area 4 keeps a list of home repair and modification resources for older adults in the Sacramento region.
  • SHRA (Sacramento Housing and Redevelopment Agency) runs home repair help for income-qualified homeowners. Programs open and close, so check what's available now.

Paying over time

Ask about payment options when you call. Whatever you do, read the financing terms carefully. "No interest if paid in 12 months" plans often charge all the back interest if there's any balance left at month 12.
